Please note that we will not typically prescribe medications such as Benzodiazepines (Xanax, Ativan etc) or hypnotic sleeping medications (Ambien, Lunesta etc) for daily use. These are short term, situational medications only
Stimulant medications may require additional labs/testing for prescribing and prescriptions for such medications are not guaranteed with first visit
Paperwork can be completed for employment, accommodations, educational concerns, emotional support animal letters, disability, FMLA, and other paperwork as needed.
A single page document will start at $25.
An additional $25 per 15 minutes will be charged to complete additional documents.
All paperwork will require 5-7 business days to complete. If you would like the form same day, it will be a $100 fee.
Medical record requests are availiable on the patient portal by request at no charge. There will be an additional fee assessed for printed records from $25+ depending on chart size
AFTER HOURS SERVICES
After-hour calls are not routinely available. If one is requested or needed and the provider decides to offer this service, an additional fee of $150/30 minutes will be charged and not covered by your insurance. All phone calls, emails and text messages will not be checked after hours and should not be utilized in an emergent or urgent situation. Please utilize the following resources:

Initial Consultation
Free 15 minutes- limited time only
Initial introduction offering an opportunity for both parties to meet. Identification of the needs of the client are obtained and the provider can offer possible options for a dynamic and individualized plan of care. No obligation to move forward for either party. Of note, no laboratory analysis or specific treatment recommendations will occur during this encounter.
Ongoing and Follow-up Care
15-60 minutes
Follow up visits for routine medication management once established as well as lab evaluation/interpretation and care planning. This will include genetic testing interpretation and recommendations which require 60-90 minutes
Initial Psychiatric Evaluation
Required for all patients, 60-90 min
1:1 session exploring your symptoms that have led you to where you are at today. Identification of an individualized treatment plan. Our advice is crafted to fit seamlessly into your routine, bringing clarity and support to your mental wellness journey.
